Treatment of Mycobacterium avium complex (MAC) lung disease as part of a combination antibacterial regimen in adults with limited or no other treatment options who do not achieve negative sputum cultures after at least 6 months of a multidrug background regimen.
Use Restrictions: Only studied in patients with refractory MAC lung disease. Patients with non-refractory MAC lung disease should not take this medication.
Adult Dosage: Only use with the Lamira Nebulizer system. In cases of known hyperreactive airway disease, COPD, asthma, or bronchospasm, consider pre-treatment with a short-acting selective beta-2 agonist. 18 years and older: Inhale the contents of 1 vial (590mg) once daily.
Children’s Dosage: 18yrs: unknown.
Boxed Warning: There is an increased risk of respiratory adverse reactions.
Arikayce Cost Warnings/Precautions:
Increased risk of respiratory adverse reactions, which may necessitate hospitalization. If hypersensitivity pneumonitis, anaphylaxis, or hypersensitivity reaction occurs, discontinue use and manage as needed. Known or suspected auditory/vestibular dysfunction; closely monitor; if ototoxicity occurs, manage and discontinue as needed. Renal dysfunction or neuromuscular disorders (e.g., myasthenia gravis) that are known or suspected; closely monitor. Monitor renal function in the elderly. Toxicology of the embryo-fetus. Avoid use during pregnancy. Mothers who are breastfeeding.

Interactions with ARIKAYCE: Avoid concomitant use with neurotoxic, nephrotoxic, and ototoxic drugs. Concomitant furosemide, ethacrynic acid, urea, and IV mannitol should be avoided. Diuretics may contribute to increased toxicity.
